Telehealth consent

By using telehealth features within the service, you consent to receive medical consultations and ongoing clinical care from licensed clinicians affiliated with AYA PCs ("Medical Provider") delivered through telehealth technology.

Telehealth services may be provided through synchronous interactions, including live video or audio consultations, and asynchronous interactions, including provider review of patient-submitted health information, secure messaging, and store-and-forward communications. The specific modality used for each interaction will be determined based on clinical appropriateness and the type of care being provided.

Your care may be provided by any licensed clinician affiliated with AYA PCs. You may not always see the same clinician for each interaction, and clinical decisions may be made by different members of the medical team based on availability and clinical need.

You understand that telehealth has benefits, including convenient access to specialized care regardless of geographic location, reduced travel time, and continuity of monitoring between visits. You also understand that telehealth has limitations, including the inability to perform a hands-on physical examination, the possibility of technology failures or delays, and the potential need for an in-person visit or referral if your condition requires evaluation that cannot be performed remotely.

You must be physically located in a state where the Medical Provider's clinicians are licensed at the time of each encounter. You will be asked to confirm your location before or during each interaction.

To protect your privacy and security, you may be asked to verify your identity through government-issued photo identification, security questions, or authenticated account access. The method of verification may vary depending on the type of encounter.

Your consent to telehealth services will be documented in your medical record maintained by the Medical Provider.

Telehealth services are not a substitute for emergency medical care. If you experience a medical emergency at any time, call 911 or go to the nearest emergency room immediately. Do not rely on the telehealth service for urgent or life-threatening conditions.

You may withdraw your consent to telehealth at any time by notifying us in writing at contact us. Withdrawal of consent will not affect your right to seek future care through alternative means, and you may request an in-person referral at any time.
